On December 10, 2014, all creditors who had checks on the “voided” list had their checks turned over to the Clerk of the Court for the United States Bankruptcy Court for the Middle District of Alabama as unclaimed funds.  Once a check has been turned over to the court as unclaimed funds, in order for those funds to be released, the owner will have to file an Application for Payment of Unclaimed Funds for Individual Owner of Record or if the creditor is deceased there is an Application for Payment of Unclaimed Funds for Other than an Individual Owner of Record .  For your ease, we have added these forms to our Forms tab on this website.  From the Forms tab you will see the Application for Payment of Unclaimed Funds for Individual Owner of Record.  If possible, fill the form out on the computer and print once completed.  You will also have to complete an IRS W-9 form (which has also been added to our Forms tab).  The form must be signed and notarized and mailed to the U.S. Bankruptcy Court along with the W-9 at the following address: United States Bankruptcy Court for the Middle District of Alabama, One Church Street, Montgomery, Alabama 36104.  Further instructions for filing the application is attached to the application on the second page.
